Diabetes Prevention Program
In partnership with University of Georgia Extension, USG Well-Being is offering the
CDC-recognized diabetes prevention lifestyle change program, PreventT2, to all staff,
faculty and spouses whether or not they are enrolled in a USG healthcare plan. The
PreventT2 lifestyle change program is a structured program developed specifically
to prevent type-2 diabetes.
This one-year small group program teaches you how to eat healthy, increase your physical activity and manage your stress to prevent or delay type-2 diabetes, with the support of a lifestyle coach and research evidence-based curriculum from the CDC. It is designed for people who have prediabetes or are at risk for type-2 diabetes, but who do not already have diabetes.
To be eligible for the program, you must:
Be 18 years or older, Have a body mass index (BMI) of 25 or greater, as well as be
considered high risk for one of the following:
•Have prediabetes diagnosed by your healthcare provider in the past year...
•Score 5 or greater on the prediabetes risk test
•Have a history of gestational diabetes
•Screen positive for prediabetes using the American Diabetes Association risk test.
