Committee Letter Requirements
Deadlines
Completed documents should be submitted by the deadlines below to ensure a timely review. Documents received after the deadline will be reviewed without a guarantee of promptness. In all cases, at least six weeks prior to the application deadline is preferred.
- June 15th for AMCAS Early Decision cycle
- August 1st for AMCAS regular decision cycle
- December 1st for all others
Students should keep in mind the deadlines of the schools you're applying to when
gathering your documents. As a student, it is your responsibility to ensure that your
pre-medical file is complete. You can check the status of your pre-medical file by
emailing the PHFLC.
Requesting a Letter
A committee letter will be composed by the PHFLC after ALL items are submitted. The
estimated timeframe for the committee letter to be completed and uploaded is estimated
at around two weeks.
Aside from the online form, all the documents below must be submitted in Word or PDF
format.

- Letter Request Forms are needed for all application services to which you are applying.
- Letters of Evaluation
- In the Letters of Evaluation section of the AMCAS application, you will need to provide the following information to generate a Letter Request Form.
- Letter Type: Committee Letter
- Author Name: Jonathan McMurry, Ph.D.
- Author Email: prehealthletters@kennesaw.edu
- Author Institution: Kennesaw State University
- Once you have submitted the above information, a Letter of Evaluation Request (PDF)
is generated for your letter authors with your information such as AAMC and Request
ID to allow letter submission. This document must be directly submitted to the PHFLC.
- Letters of Recommendation
- Minimum of three (3) Kennesaw State science or related discipline faculty recommendations, preferably from more than one discipline. Must be on KSU letterhead.
- Minimum of two (2) outside references from other sources, e.g., physician, volunteer
coordinator, etc. Professional Verified Application Report (AMCAS, AACOMAS, AADSAS)
or copy of time-stamped submitted application for other schools. This is the processed/verified
paper copy of your professional application or a PDF form that you will turn in. Letters
must be on company letterhead or written using a business-style format that includes
a heading.
- Letters do not have to be sealed when turned in by the recommender.
- Only recommenders can e-mail letters as attachments directly to the PHFLC.
- Very Important: When writing a letter of recommendation, do not address letters of recommendation to Kennesaw State University, the KSU pre-health faculty committee, or any KSU department. The student is not applying to KSU. The student is applying to multiple health schools across the country; therefore, the letter should be addressed generically to “Admissions Committee” or “Selection Committee," and if mentioning the school in the body of your letter, it should be mentioned generically as “medical school” or “your dental program" or “your institution.
- (Post-bacc and graduate students only) Transcripts of undergraduate coursework and
degree earned if undergraduate coursework was not completed at KSU.

Pre-Health Letter Checklist
To obtain a composite committee letter from the KSU PHFLC, you must complete a pre-medical
file with the college. The items listed on the checklist below are required to establish
your file. Only fully completed documents will be reviewed by the committee.
- Document Information Form with signed FERPA Waiver and Restriction Form
- Verified application report
- AMCAS Letter of Evaluation Request (AMCAS only; AADSAS or AACOMAS will email the user’s advisor.)
- Three KSU science or related discipline faculty recommendation letters on letterhead.
- Two recommendation letters from other sources.
- An institutional GPA of 3.5 or higher.
- (post-baccs or graduate students) Transcripts from other institutions attended.
